ACT Team Lead

ACT is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The ACT model is characterized by a team approach, shared caseload, time-unlimited services, flexible service delivery, and 24/7 crisis availability. 

The ACT Team Leader provides direction, guidance, and feedback to each member of the team, including providing clinical supervision and additional training when appropriate. The Team Leader will also participate in treatment planning, crisis evaluation, therapy provision, and coordination of services. 

The Team Leader will also have administrative duties that include coordinating admissions & discharges, tracking clients’ treatment progress and access to care within the program, ensuring staff’s timely & thorough completion of clinical documentation, and coordinating staff schedules.

MAJOR DUTIES AND RESPONSIBILITIES

The main duties for the position shall be to create and maintain the following:

  1. The ACT Team Leader is the coach for the ACT Team, providing both clinical and organizational leadership, team building, appropriate utilization and quality assurance, ensuring the involvement of all team members in all aspects of the teams work. 
  2. As part of a multi-disciplinary, core services team, provides assessment and direct service to people with seriously and persistently mentally illness who are enrolled in the ACT Program.

The specific responsibilities shall include the following:

  1. Participates:
    1. As a team supervisor in the frequent monitoring and assessment of the mental health status of people served as well as related variables, including significant others, the home and the community in which the person served lives.
    2. In the development of comprehensive assessment, coordination of all other assessments, and an individualized service plan.
    3. Lead the clinical and non-clinical team meetings, including individual and team supervision.
  2. Create and Maintain:
    1. A wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning, and evidence-based practices. All services provided will promote a wellness and recovery outcome.
    2. A culture of compliance with internal and external policies, regulations, laws and high ethical standards.
  3. Provides and Monitors:
    1. Clinical supervision and direction to the ACT team, emphasizing a peer clinical supervision approach and using individual supervision as needed
    2. Rapid and flexible response to crisis, including but not limited to: accompanying person to local screening center or psychiatric emergency service and remaining with the person during the assessment process. 
    3. 24 on-call coverage at all times. Assessment of individuals’ daily living skills, and teach individuals needed skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy and ways to maintain housing. 
    4. Support to assist individuals to find and maintain employment and/or educational training.

The Physicians who diagnose, treat and help prevent diagnose, treat, and help prevent disorders of the mind.  They assess and treat mental illnesses through a combination of psychotherapy, psychoanalysis, hospitalization, and medication.

Responsibilities:

Prescribe, direct, or administer psychotherapeutic treatments or medications to treat mental, emotional or behavioral disorders. Analyze and evaluate patient data or test findings to diagnose nature or extent of mental disorder. Collaborate with physicians, psychologists, social workers, psychiatric nurses, or other professionals to discuss treatment plans and progress. Design individualized care plans, using a variety of treatments. Gather and maintain patient information and records, including social or medical history obtained from patients, relatives or other professionals. Counsel outpatients or other patients during office visits. Examine or conduct laboratory or diagnostic tests on patients to provide information on general physical condition or mental disorder. Advise or inform guardians, relatives or significant others of patients’ conditions or treatment. Teach, take continuing education classes, attend conferences or seminars, or conduct research and publish findings to increase understanding of mental, emotional, or behavioral states or disorders. Review and evaluate treatment procedures and outcomes of other psychiatrists or medical professionals. Prepare and submit case reports or summaries to government or mental health agencies. Serve on committees to promote or maintain community mental health services or delivery systems. Qualifications: Four (4) years of undergraduate school, four (4) years of medical school, and three (3) to eight (8) years of internship and residency, depending on the specialty selected.  A few medical schools offer combined undergraduate and medical school programs that last six (6) rather than the customary eight (8) years. Possess the amount of training and/or a degree from an accredited school of medicine.  This usually requires four (4) years of pre-med and four (4) years of medical school. Complete an accredited program of psychiatry. Pass a national exam. Apply for licensure in the state you wish to practice in. Premedical students must complete undergraduate work in physics, biology, mathematics, English, and inorganic chemistry.  Students also take courses in the humanities and the social sciences. Some students volunteer at local hospitals or clinics to gain practical experience in the health professions. ADDITIONAL QUALIFICATIONS:  Participates in any Agency required training.  Must be free of the communicable disease of tuberculosis and agree to an initial PPD testing. City Care Health Services is seeking dedicated individuals, who want to help facilitate change and growth, to work with adults and children that have serious mental illness and disabilities as community support workers/case managers.  Community support workers spend at least 70% of the time in the field teaching clients the skills required to manage their disorder and succeed in their individual recovery goal.  Community Support Workers support individuals in attaining meaningful, sufficient lives through the growth of the skills necessary to live independently, acquire employment, complete their education/training and have vigorous social lives.

Responsibilities:

Assisting people in identifying personal recovery goals. Assisting with community resources such as housing, food banks and medical care. Providing psycho-educational and teaching symptom management skills to minimize the effects of symptoms of individual daily lives. Helping people develop their personal support systems. Knowledge, Skills, and Abilities: Strong oral and written communication skills Strong organizational, computer, and time management skills required Previous work in the mental health field and/or social services/social work, preferred Must pass background check.
