ACT Team Lead

ACT is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The ACT model is characterized by a team approach, shared caseload, time-unlimited services, flexible service delivery, and 24/7 crisis availability. 

The ACT Team Leader provides direction, guidance, and feedback to each member of the team, including providing clinical supervision and additional training when appropriate. The Team Leader will also participate in treatment planning, crisis evaluation, therapy provision, and coordination of services. 

The Team Leader will also have administrative duties that include coordinating admissions & discharges, tracking clients’ treatment progress and access to care within the program, ensuring staff’s timely & thorough completion of clinical documentation, and coordinating staff schedules.

MAJOR DUTIES AND RESPONSIBILITIES

The main duties for the position shall be to create and maintain the following:

  1. The ACT Team Leader is the coach for the ACT Team, providing both clinical and organizational leadership, team building, appropriate utilization and quality assurance, ensuring the involvement of all team members in all aspects of the teams work. 
  2. As part of a multi-disciplinary, core services team, provides assessment and direct service to people with seriously and persistently mentally illness who are enrolled in the ACT Program.

The specific responsibilities shall include the following:

  1. Participates:
    1. As a team supervisor in the frequent monitoring and assessment of the mental health status of people served as well as related variables, including significant others, the home and the community in which the person served lives.
    2. In the development of comprehensive assessment, coordination of all other assessments, and an individualized service plan.
    3. Lead the clinical and non-clinical team meetings, including individual and team supervision.
  2. Create and Maintain:
    1. A wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning, and evidence-based practices. All services provided will promote a wellness and recovery outcome.
    2. A culture of compliance with internal and external policies, regulations, laws and high ethical standards.
  3. Provides and Monitors:
    1. Clinical supervision and direction to the ACT team, emphasizing a peer clinical supervision approach and using individual supervision as needed
    2. Rapid and flexible response to crisis, including but not limited to: accompanying person to local screening center or psychiatric emergency service and remaining with the person during the assessment process. 
    3. 24 on-call coverage at all times. Assessment of individuals’ daily living skills, and teach individuals needed skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy and ways to maintain housing. 
    4. Support to assist individuals to find and maintain employment and/or educational training.

Qualifications: LICSW or/LPC or /RN with a Graduate Degree
